We provide comprehensive Case Management in all areas of rehabilitation for clients, their families and lawyers.

We are committed to helping individuals recover as much as possible and enabling them to live life to the full again.

Our service is designed around the client’s specific needs to ensure they live their life in their way. We provide specialist care for adults, young people and children with:

  • Brain injuries
  • Spinal injuries
  • Cerebral Palsy
  • Orthopaedic injuries
  • Amputation

Due to the varied background of our carefully chosen Case Managers, it is likely that there will always be an appropriate expert to address the individuals’ needs.
Within AKA Case Management, our processes and structures are designed so that each client and legal team receive the full benefit of the experience and knowledge within the AKA team.
We undertake to achieve the following:

  1. Provide a specialised, uniquely tailored, community based caring service to adults disabled by the multiple effects of brain injury, spinal cord injury, orthopaedic injuries and children with cerebral palsy, which is professional, effective and fun to be part of.
  2. Provide highly trained care staff capable of implementing innovative support programmes within the client’s home that promotes an independent meaningful life for that person.
  3. Create a caring, supportive community in which all those part of AKA Case Management Limited and associated with it feel valued, respected and included.
  4. Create an innovative, compassionate, ethical business that has a sound solid business structure which endures.
  5. Provide fund holders (Lawyers, Deputies, Primary Care Trusts, clients and their family/significant others) with a quantifiable best practice/best value for money service.

AKA provides personnel in the following roles:

  • Case Managers
    • The Case Manager will work alongside you getting to know the client and how their injury has affected their way of life. In this way they can ensure things are in place to help the client overcome their disability and get back to living the life they desire.
    • They will work at the client’s pace respecting their views and wishes at all times.
    • They are responsible for undertaking the needs lead assessment writing, the case management plan and support/care plan which takes into account the client’s choices and requests.
  • Assistant Case Managers
    • These people by their name assist the Case Manager where required; for instance if the client has therapy support workers/carers they will manage the team ensuring the staff provide the highest possible standard of support/care.
  • Therapy Support Workers/Carers
    • These people will be with the client for the hours dictated by the support/care plan.
    • They will perform duties agreed by the client in advance and undertake therapeutic activities as directed by the client’s therapist.
    • Therapy Support Workers enable the client to access the wider community and will support in social and leisure pursuits too.
  • Other Specialist Professionals
    • To ensure all needs are met we contract with other specialist professionals to provide other services such as physiotherapy, occupational therapy, speech and Language therapy, Neuropsychology, Information Technology and many forms of alternative therapies and services.
