Northern Case Management (NCM) was established in 1997 to provide support to Children and Adults with acquired neurological damage (i.e. brain, spinal cord injury) and empower them to develop their full potential and enrich their lifestyle through providing a comprehensive client focused service.

The primary role of the Case Manager is to work alongside the Client and their family / representative to identify areas of need and agree on an action plan to meet their rehabilitation goals and bring about an improvement in their quality of life. The Case Manager will work with the Client to set SMART goals – Specific, Measurable, Achievable, Realistic and Time bound, ensuring that intervention is Client focused.. In partnership with the Client, the case manager will identify appropriate therapists and professionals to work with the Client to meet, and hopefully exceed the Client’s rehabilitation goals. Our work includes facilitating the client’s and family’s adjustment to the injuries; recruiting, training and supporting support workers; and managing care regimes.

NCM continues to meet the increasing requirement for case management services and now boasts a multi-disciplined team of 8 case managers plus support staff and manages over 200 support workers on behalf of Clients. Our case managers are fully trained and registered health professionals including nurses, Occupational Therapist etc and come with a wealth of experience and clinical knowledge.

At NCM we also provide training services with our trainers providing in house mandatory training for staff that can be completed at our head office, in the clients’ homes or other suitable venue. Offering flexibility to meet the care teams requirements and involving the client and their family.

Our team of occupational therapists carry out a range of thorough assessments to design the rehabilitation programme using activity as a vehicle to achieve improvement. This also includes equipment and accommodation assessments to maximise the client’s quality of life and independence.

NCM is determined to continually develop in order to provide intervention for as many people as possible, and it’s always looking to increase the size of our team.
