Uplands Rehabilitation Centre in Westcliff-on-Sea is a recently refurbished state of the art facility that has been designed and purpose built to be able to provide tailored, individual rehabilitation programmes for up to 17 adults with an acquired brain injury or neuro-disability.
The Centre provides 24 hour nursing care and therapy services for 17 adults with complex neurological conditions with the overall objective to reduce disability and improve an individual’s social and emotional well-being in order to maximise the future prospects of a more fulfilling life or, a return to more independent living. It is able to accommodate residents with tracheostomies and / or those who are ventilated.
There is an in house multi-disciplinary team consisting of professional nurses, dedicated rehabilitation support assistants, occupational therapist, physiotherapists, speech and language therapist and psychotherapist; all supported by two local GP practices as well as having access to numerous professional external bodies including a Consultant Neuropsychiatrist and Consultant Neuropsychologist.
Time specific placements are also offered, including programmes which prepare for a return to more independent living; including practising day-to-day life skills and implementing transitional arrangements to ease re-integration into the community.
Uplands uses the NPDS and sets / measures progress of goals / outcomes using GAS, Barthel and FIM / FAM.
